R.A.I.N. case managers serve homebound Bronx residents age 60 and older in Community Districts 7, 8, and 12, coordinating transportation, housekeeping, home care, meals, and benefits. Its city-funded meal program offers eligible residents daily hot delivery or twice-weekly fresh-frozen delivery after assessment by a NYC Aging-funded case management agency.
