Sonoma County's Aging and Disability Resource Hub connects older adults, people with disabilities, and caregivers to benefits, meals, transportation, caregiver support, case management, adult day care, and in-home service pathways. County social workers provide free phone help in English and Spanish, with interpretation or communication accommodations arranged for other languages upon request.
