Sonoma County Aging and Disability Resource Hub

Sonoma County's Aging and Disability Resource Hub connects older adults, people with disabilities, and caregivers to benefits, meals, transportation, caregiver support, case management, adult day care, and other services through the Adult and Aging Division as Area Agency on Aging. Phone help is available in English and Spanish, with interpretation and communication accommodations by request.

Use the hub when you need a county front door before committing to a specific day program or meal provider. Referrals do not guarantee program eligibility or availability.

Call with the person's age, city, and main need so the referral can be specific enough to act on.
