Sourcewise Care Management and Meals on Wheels

Sourcewise connects Santa Clara County older adults with information, care management, benefits resources, and home-delivered meals. Its MSSP team includes a social worker and registered nurse for eligible participants who meet age, Medi-Cal, residence, and risk criteria.

Meals on Wheels has separate homebound, age, residence, and functional criteria from care management. A Community Resource Specialist is the countywide contact when you are not sure which door fits.
