Contra Costa County Nurse-Family Partnership

Contra Costa County pairs eligible first-time mothers with a public-health nurse for ongoing home visits from early pregnancy through the child's second birthday. Eligibility is stated as first-time mothers at 28 weeks or less who are eligible for Medi-Cal or WIC.

The program documents individualized goal planning, life coaching, education, and self-referral. If you do not meet NFP criteria, the county can screen for other home-visiting support.
