JASA Care Transitions

JASA's Care Transitions program supports adults with complex health and social needs as they move from hospital to home or face community risks. Staff help coordinate food, transportation, housing, legal, caregiver, and benefits resources. Services are explicitly offered in English, Spanish, Chinese, Russian, Haitian Creole, French, Hindi, Urdu, Punjabi, and Igbo, subject to intake criteria.
