Los Angeles County Public Health runs Nurse-Family Partnership as a free, voluntary nurse home-visiting program. Specially trained public health nurses support eligible first-time pregnant people beginning in pregnancy and can continue until the child turns 2.
The model is built for higher-need, lower-income first-time parents, not as a short postpartum check-in. Referral runs through a confidential form and email to HomeVisit@ph.lacounty.gov, with MCAH office phone (213) 639-6478.
Start the referral during pregnancy if you want the full visit schedule. This is a long relationship with a nurse, so treat enrollment timing as part of prenatal planning.
Cost: Free and voluntary
